"The Walking Dead" season 4 episode 5 titled "Internment" features this quote (taken from the clip) - "Not everyone gets to live," clearing pointing towards more deaths in the next Sunday's episode.

In "Indifference," the trip taken by the group turned out to be one hell of a ride. Tyreese almost lost it, and the group was attacked by the walkers and Rick sent Carol back. But, these are nothing compared to what the prison mates are suffering, they are bleeding from their eyes as they desperately wait for medicines.

The gates of the prison will keep the zombies out, but the infection that is prevailing in the prison is killing people. Even, Hershel sitting next to the patients will do them no good.

The preview of "The Walking Dead" season 4 episode 5 points out that Rick, Daryl and Michonne (with Tyreese and Bob) are back in the prison with the medications, but it might not have happened in time to save all the infected people with the medicines they have. Things are not great for Caleb. But, we don't know if his advice to Hershel to let the ill ones die will be with the man as he tries his best to treat the patients.

In "Indifference," Carol did stab David and Karen to their deaths. It was done to put them out of their misery and also to stop the disease from spreading in the prison. Rick sent Carol packing because if she stays in the prison then Tyreese would come after her and kill her. Plus, he wasn't interested in having Carol around his kids.
